Appointments

Appointments are scheduled through our main office number 301-770-GYNS (4967). If you currently have a scheduled appointment, please download and complete the registration packet prior to the appointment.

Next, please enter our patient portal to enter your complete medical history.

You should have received a letter issuing you a user name and password to enter the portal. If you have not received this letter, please call us at 301-770-4967 and our appointment representatives will be happy to provide you the information. It is essential that this information is completed before your visit.Your appointment may need to be rescheduled if the information is not present at time of service. It is necessary for you to provide copies of appropriate medical records for your visit. Please also bring identification and your insurance cards to complete your registration.

If you need a copy of your medical records, please print, download and complete the Medical Records Requests Form. There will be an administrative fee for medical records duplication. You may fax the form to our office at 301-770-3205.

Your First Visit

Please arrive 10-15 minutes prior to your appointment to fill out the necessary patient information forms. Please be sure to have your insurance card or reasonable facsimile from your insurance company's website and a list of all medications you are currently taking. Our goal is to try to stay on schedule but unfortunately sometimes unforeseen circumstances may arise and we thank you for your patience.

On your first visit (as well as any other visit) it is a good idea to bring any historical information about your health and a list of questions so your doctor can get to know you better and be aware of past issues or concerns you may have. Our job is to have an honest and open relationship with you.

Obtaining Medical Records

If you would like a copy of your medical records, please print, download and complete the Medical Records Requests Form. There will be an administrative fee for medical records duplication. You may fax the form to our office at 301-770-3205. Please allow ten business days for processing.

Insurance & Payment Policies

Our physicians participate with Medicare and accept assignment. We are contracted with most major insurance carriers. Your payment or co-payment is due at the time of service. We accept cash, check, credit card: Mastercard or Visa. As a courtesy to our patients we will submit the bill to your insurance carrier. Please bring all plan ID cards so your claims may be filed with the correct carrier.

If you are a member of an insurance plan with which we participate, we will follow their billing requirements. If a referral authorization is required, please obtain it prior to your visit.
